Sacred Hearts Academy is pleased to serve as a host site for an upcoming CVS / Longs Drugs Drive-Thru Vaccine Clinic on Saturday, September 5.
Individuals and family members with HMSA, Medicare Prime, UHA, HMAA, or TRICARE may drive onto the Sacred Hearts Academy campus and receive up to three vaccines without leaving their vehicle.
- General Public | 9:00 a.m.–2:00 p.m.
Those interested may register by scanning the QR code on the attached CVS flyer.
- Special SHA ʻOhana Hours | 8:00–9:00 a.m.
A special one-hour time slot has been reserved exclusively for Sacred Hearts Academy families and friends.
To take advantage of this designated time, please complete a Longs Drugs Consent Form for each individual receiving a vaccine and bring the completed form with you on September 5. Anyone who has Medicare would also need to fill out the attached Assignment of Benefits Agreement and Consent Form.
As an added benefit, those receiving vaccinations will receive a $5 off $20 coupon and a FREE swag bag.
